F2A: Supporting Successful Transitions: Introduction & Implementation of the BRIDGE Transition Toolkits

AbstractThis presentation will guide attendees through the transition process utilized the Florida Atlantic CARD BRIDGE Toolkits, designed to support individuals with autism as they move from high school to adulthood. Emphasizing the development of self-determination, self-advocacy, and independence, this session aims to equip participants with the knowledge and tools necessary to facilitate successful transitions. Key focus areas will include essential skills for independent living, community safety, post-secondary education, and employment. Strategies for implementation can be utilized in a classroom for teachers or in the community by families. Additionally, attendees will gain insight into available resources and support systems for students with autism throughout their later school years, as well as strategies to prepare them for life beyond high school. Florida Atlantic CARD bridge initiative provides guidance and support to transition-age students 12–25 years of age with autism spectrum disorder parentheses ASD parentheses or related disability, their parents, and families. Bridge stands for building resources for independence through self-determined growth experiences. The transition road from youth to adult is a tremendous change. The bridge initiative is here to provide expanded specialized support and consultation to develop meaningful Outcomes for adulthood. The initiative has three pillars. The first pillar offers direct support for transition, focused individuals, their families, and support teams. The second educates constituents, families, and community partners. The third provides interactive services and opportunities for transition-age youth and adults to build social skills, self-advocacy skills, and vocational skills while developing adult goals. Supports include adult hangout groups, advisory council, and the Art and autism expo.
Objectives1. Attendees will be able to explain the core skills of transition for individuals with disabilities and their families.  
2. Attendees will be able to describe strategies to support students in identifying their areas of need in the transition to adulthood.  
3. Attendees will be able to apply concepts from the BRIDGE Transition Toolkit to empower students of transition-age.
